House training older toy poodle

I have a 6lb toy poodle that is about 6 years old. Got her from a breeder. I believe the dog was kept in a cage and probably just peed and pooped in the cage. I can't get her to stop peeing in the house. I have to watch her constantly. She will pee when taken outside but then maybe 20 minutes later she'll pee in the house. she's even peed on the bed at night. I just don't know what to do. I take her outside at least 3-4 times in the evening. Please, anyone know how to break her of this? Should I crate her at night and if so, what size crate?

Yes crating her whenever she can not be watched by someone is a must, UNLESS as you said she lived in a crate and learned to go to the bathroom in that. If that's the case then the crate will not help with housebreaking. If you use a crate the crate should be large enough for the dog to stand up lay down stretched out and turn around in. If it is too big then the dog will go to the bathroom in it anyways. if he soils the crate then I'm not sure how to go about training and you'll need someone else's advice on that.
