You just have to watch the measurements. For example, I know Luna wears an 8-inch collar, so if I'm buying one online and it doesn't say 8-inches, I don't buy it. Small, x-small, xx-small, they're all different depending on the manufacturer. She wore a medium once, and she's worn xx-small before too.

I go by measurements, rather than sizes.
Emma (my smallest at 3lbs) has a 6" neck and 6" length. When I order stuff for her, I make sure measurements are listed.
And be careful about buying anything "over the head." Puppia harnesses don't fit her, since the XS are too small to get over her apple head, and the size S are swimming on her. I buy her harness vests from Chloe's Four Legged Fashions.
I made a thread a few weeks ago about a site called Fetch Dog. They were (not sure if they still are) clearing out dog jackets, sweaters, collars, etc. They have coats meant to fit up to 3lb dogs, which was right up Emma's alley!
