It depends on where you live. Many states do not require certification at all - especially since there are a few people who prefer to train their own service dogs.

Here's a great site that will tell you anything and everything you need to know about service dogs, assistance dogs, guide dogs, pilot dogs... basically all dogs that help people!

Depends where you live

Over here, the certified assistance/service dog trainer after a several evaluations can certify the dog as an assistance/service dog if it has met strict criteria in several different styles of tests, im not sure about other places.

A great website to look at is "Assistance Dog International"s website

Here in NY I do know they have to be evaluated and tested in order to be certified. There is a gal who went to the obed. school we go to who is in a wheelchair. She got a Portugese Waterdog and trained him to be her service dog. She also competed with him in obedience. She did say that after he was certified he will have to be re-evaluated at periodic intervals to retain his certification. I'm not sure if it is yearly or every 2-3 yrs.
