Essential Puppy Things?

Personally, McGee and I have really liked the Petstages Puppy Cuddle Pal. It has a little sack of buckwheat tucked into a velcro pouch. You can microwave the whole toy for 30-45 seconds and it stays warm for about 45-60 minutes. That's long enough to feel like a littermate while she falls asleep, and keeping the tiny girl from getting chilled in my drafty house. I found mine at Petsmart for about $8 or $9.
Other than that -- I second what has been said. Food and food dishes, collar and leash, crate, blankies, toys. Also, though, don't forget to think ahead about treats for potty training, which I had to cut up much tinier than I originally thought. Most recommend hot dogs, mozzarella cheese, etc.
Today I am going to buy a bell to hang next to the door to help us with potty training. That's the one thing I didn't get before she arrived that I wish I had.

Oh, you'll probably forget something... don't worry about it. The puppy will live. 
In the meantime, when you realize what you've forgotten, post it so that the next guy will remember it. Hee!
Oh -- have you been able to take a tiny blanket or one of your old t-shirts for the breeder to keep with the pups until you bring him/her home? I think that also really helped McGee. Milo (my older dog) and I slept with a little blanket for a few days, then took it to the puppies' house. After it had been with the litter for about a week, I went to visit again. I swear McGee was happier to see me that time than she had ever been.

That's what I've been doing and it's worked fine. I suppose you could take out the pouch and microwave that every time, but seems like it'd be annoying after a while. When I take her out in the middle of the night, I take the toy out of her crate. I stick it in the microwave when we go outside. It's ready to go back into her crate with her when we return to my room. Perfect. It really does help her quiet down.
With my experience of buckwheat in the past, after a while, it doesn't hold heat as well. I guess you could replace the little pouch, if you wanted to keep using it. Wouldn't take too much sewing skill to make a little bag of rice or wheat.
